Usage: mdihi <database> <instance> <patient resource id> <ihi>

Example: mdihi HCN_SAMPLES server1\HCNSQL07 23 8003608233326545

mdihi /patients HCN_SAMPLES SERVER\HCNSQL07

Details:
- samples HCN_SAMPLES
- live HCN
- instance name HCNSQL07 
- Patient.id identifies patient

/namefile text file format; 1 entry per line - <lastname>[,<firstname>] <ihi>
e.g.

list all patients
